Chemistry: internship - risk analysis

The risk analysis will be completed by the internship provider and signed by the internship provider and by the occupational physician of the internship provider. In case your internship provider doesn’t have an occupational physician, the internship provider will take full responsibility for the risk analysis.

 The internship provider must indicate if a health assessment is needed or not, and if they will organize that or if they will delegate it to UAntwerpen (via Mensura).
